Abstract:BACKGROUND: Detailed characterization of the molecular tumor profile is a prerequisite for personalized oncology approaches. This profile is dynamic in nature and requires sequential analyses to closely monitor the patient during and after therapy. OBJECTIVES: The goal was to investigate the potential of liquid biopsy analyses for improving diagnostics, therapy and follow-up of patients in pediatric hematology and oncology. MATERIALS AND METHODS: The basic literature reflecting technology advances, preclinical and clinical studies is reviewed and expert opinions are presented. RESULTS: The analysis of tumor surrogates in liquid biopsies such as blood, saliva, cerebrospinal fluid and urine is less invasive than surgical biopsies and can be sequentially performed. The high temporal resolution achieved allows the likely detection of changes in the genetic fingerprint of a cancer. Initial studies indicated that liquid biopsies are capable of capturing the landscape of intratumoral heterogeneity in the primary tumor and the metastases at the (sub)clonal level, thus, outperforming the profile provided by a local surgical biopsy. In pediatric hematology and oncology, cell-free tumor DNA is currently the most studied molecule in liquid biopsies. Further tumor surrogates in liquid biopsies include RNA molecules, circulating tumor cells, extracellular vesicles, metabolites and proteins. CONCLUSIONS: The technological advances of the last decade now enable the detection and characterization of the dynamic fingerprint of solid pediatric tumors in liquid biopsies. The potential of these analyses for initial diagnostics, disease monitoring, therapy decision-making and early detection of relapses during follow-up is currently under intense investigation within the framework of clinical studies in pediatric oncology.
