Abstract:In this joint statement of the German Cardiac Society (DGK) and the German Society for Occupational and Environmental Medicine (DGAUM), electromagnetic interference of electrically active cardiac implants are discussed, the available literature is summarized and assessed as far as possible. Due to the increasing technologization of the daily routine and the occupational environment with increasingly more electrical and electronic devices, this topic is of increasing relevance. This is accompanied by further developments of existing transvenous implants and novel aggregate technologies, such as cordless pacemakers and total subcutaneous implantable cardioverter defibrillators. The configuration of electromagnetic fields (EMF) can greatly vary with respect to frequency and field strength and elicits very different reactions in the body and therefore in aggregates. The physical background of the effect of EMF on the body and implant is demonstrated in a generally understandable way. Various interference scenarios are elucidated with respect to the underlying disease. From interdisciplinary perspectives, aspects of exposure to EMF in the daily routine as well as in the occupational environment are illuminated. The primary care of implants in patients exposed to EMF, risk assessment in the context of occupational medicine and device programing with trouble-shooting are of particular importance. In addition, concrete approaches for occupational physicians in businesses are provided in order to support the reintegration process. This includes a brief description of the legal situation. In this way the document aims to motivate the initiation of studies that systematically investigate the topic of electromagnetic interference in vivo for current aggregate generations and new developments, so that a sufficient risk assessment can be carried out for various frequency ranges and therefore technical devices.
