Abstract:Secondary immunodeficiencies (SID) attribute considerably to the morbidity and mortality of hematological and oncological patients. The early detection of secondary immunodeficiencies by means of the following clinical characteristics - infections with opportunistic pathogens - recurrent infections with common pathogens - atypical infection sites, polytope infections - prolonged and severe infections - increased total number of infections as well as the initiation of deliberate preventive strategies is essential in the treatment of patients with a hematological or oncological disease. Furthermore, the knowledge of any pre-existing illnesses, the stage of the underlying disease as well as the number and kind of preceding therapy regimens is crucial in the clinical evaluation of SID. When suspecting SID, full laboratory work-up, particularly a quantification of immunoglobulins should follow. When SID is diagnosed, preventive measures should be implemented, e g immunoglobulin substitution therapy when hypogammaglobulinemia 4 g/l is detected.
