Abstract:Important recent insights into tumor pathogenesis provide a rational basis for the development of novel therapeutic strategies that efficiently target genetic defects in malignant tumors. In contrast to conventional DNA damaging therapeutics, such small molecule inhibitors or monoclonal antibodies allow for a far more selective, targeted therapy of tumors that carry a corresponding target structure or gene expression profile. Such target structures are often receptor tyrosine kinases, i.e. components of the EGF-receptor signaling machinery in carcinomas. Novel targets include the PI3-/Akt-kinase/mTOR pathway and regulators of cell survival and cell death. Molecular therapeutics are only active in tumors carrying the corresponding target structure. Targeted therapies therefore necessitate the clinical deployment of adequate molecular diagnostics for the identification of those patients who have a chance to benefit from these novel targeted therapies.
