Abstract:	In Germany acid-base status is not determined in routine blood (serum) tests (CO2 content). Thus, clinicians need other specialized tests, such as blood-gas analyses. These are unfortunately commonly made on venous blood samples. Attention is drawn to the distinction between acid accumulation and bicarbonate losses and the relevance of calculating the anion gap is explained. This article provides two patient-oriented examples where clinical decisions would rely on the principles developed. Finally, the fact that any therapeutic strategy must involve immediately stopping the primary mechanism of acidosis is underscored.
