Abstract: | Axillary lymph node dissection is a standard procedure in surgical treatment of primary breast cancer. It is staging as well as therapy. With a negative status of the lymph nodes this is, however, an excessive therapy and does not warrant a morbidity of 10-30% as consequence of this treatment. The detection of potential lymphogenic metastasis in the first draining regionary lymph node (SN) is also possible using blue dye or radionuclide. The ex-ploration and histological examination of this lymph node can lead to a prediction of axillary lymph node status with 97% accuracy. Patients without metastasis of the SN can therefore in principle have the possibility of avoiding a conventional axillary dissection. In how much this can be generally indicated or with additional criteria are required is currently subject to prospective trials. |
