Abstract:Increasing life expectancy is leading to a growing number of older patients with vasculitides. Giant cell arteritis and ANCA-associated vasculitides are particularly prevalent in advanced age and are characterized by multimorbidity, age-related organ vulnerability, and altered pharmacodynamics. Key pathophysiological mechanisms include immunosenescence and inflammaging, which, through immune dysregulation, chronic inflammation, and endothelial dysfunct ion, promote the onset and progression of autoimmune-mediated vascular inflammat ion. Management therefore requires an individualized, interdisciplinary approach that balances efficacy and risk of adverse events. Steroid-sparing strategies, supportive measures, and the integration of geriatric concepts are crucial to preserve functionality and quality of life in older adults. Future studies should specifically include elderly cohorts.
